Anxiety is a childhood survival strategy (think your way out of danger) that gets baked in, and it lives not just in the mind but in the heart/autonomic nervous system and gut microbiome - so treating only the mind explains why therapy fails some people.

Anxiety is not only in the mind
- Anxiety is a heightened baseline of worry around projected threats: the calibration between real and perceived threats is broken, so perceived threats get moved into the "real threat" column and the body responds accordingly.

Origin: a five-year-old's survival strategy
- Anxious adults usually had a childhood that taught them to worry (example: an alcoholic parent). A young child has no power - cannot leave, cannot set boundaries, cannot negotiate - so the only tool is to think through every scenario and plan contingencies.
- The brain learns this during the formative period when it is also learning how the world works, so "I can't fix the problem, I can only think my way through it" becomes the default. Carried into adulthood, the person invests huge energy for tiny control - a bad trade, but the only one the brain knows.
- This becomes self-fulfilling: not acting causes problems, but the anxious person is decent at damage control, so the brain concludes "thank God we planned - planning leads to survival." Explains why someone can lie in bed doing nothing while ruminating endlessly, and why "why don't you just email your professor" feels impossible.
The heart and autonomic nervous system
- The heart adapts its rate to stress; in anxious people that adaptability (heart rate variability) is blunted - they run at high alert at baseline, same pattern seen in PTSD. Researchers can predict emotion from heart rhythm with ~75% accuracy.
- Body and brain move in concert: adrenaline makes perceived threats feel more real and forces focus onto the threat. What looks like distractibility is really the mind being hell-bent on the threat. Controlling heart rate / the autonomic nervous system (e.g. exercise) reduces anxiety.
The gut microbiome
- Gut bacteria heavily influence anxiety. IBS and inflammatory bowel disease co-occur with depression/anxiety; depression and anxiety are partly inflammatory conditions - giving IBD patients an immunomodulatory (immune-suppressing) agent measurably improves anxiety.

- Anxiety drives an urge to control the situation (get a partner to promise never to leave), because controlling the situation controls the anxiety - but you cannot control other people, so you loop forever. Instead, disconnect the situation from the anxiety and build tolerance.
- Tolerance via exposure therapy (evidence-based for phobias): enter the anxiety-provoking situation without controlling or escaping it, and wait for the body to return to homeostasis. Start small (five minutes, no phone). Avoidance controls the situation and prevents tolerance from ever developing.
- Watch the anxiety-to-behavior tie (pleading, crying, avoiding) - a central CBT principle is not acting on the safety behavior immediately.
- Detachment: anxiety usually involves the ego ("this person won't like/respect me"). Fear of displeasing others makes people doormats who cannot set boundaries. Separating from the ego - "so what if this person dislikes me" - is the antidote; go back and find where you learned you cannot afford to piss anyone off. Mindfulness and psychedelics research suggest their mechanism is separation from the ego.
